Financial Information

Our doctors and staff are proud to be a team whose primary mission is to deliver the finest and most comprehensive periodontal services available today. We are concerned about your dental care and want to ensure you that it is performed in the most responsible manner. In order to assist you with the investment in your dental health, we have outlined our payment policy.

Payment schedule is as follows for the following procedures;

Periodontal Maintenance: If you have insurance, we will submit first and bill you. Payment is due within 90 days. If you do not have insurance, payment is due in full at the time of service.

Evaluation: Payment is due at time of service. If you have insurance, we will also submit for you.

Scan/xrays: Payment is due at time of service. If you have insurance, we will also submit for you.

Scaling and Root Planing: We will collect 1/2 of the total and submit to insurance if applicable. You have 90 days to pay the remainder.

Biopsy: We will collect 1/2 of the total and submit to insurance if applicable. You have 90 days to pay the remainder.

Surgery: If you have insurance, we will collect 1/3 of the total and submit to insurance. You have 90 days to pay the remainder. If you do not have insurance, we will collect 1/2 of the total and you will have 90 days to pay the remainder.

For your convenience we accept cash, checks, money orders, and credit card payment (MasterCard, Visa, and Discover). We also offer a 6 months. no-interest financing plan with Care Credit as a third party financial option. For all services we provide a 90 day no interest financing in our office, along with the courtesy of a monthly statement.

Payment is expected at the time the services are rendered. You are responsible for knowing your insurance coverage and limitations of your plan (such as yearly maximums and deductibles). We are OUT OF NETWORK with your dental insurance company so please confirm that you are free to see the provider of your choice. We are not a medical office and therefore do not submit to medical insurance. You have 90 days to pay your bill regardless of the status of your insurance claim. If the entire balance is not paid within 90 days, a service charge will accrue monthly. Please be aware, we are not a Medicare/Medicaid provider. We accept Visa, MasterCard, Discover, Care Credit, Checks and Cash. We kindly ask for 24 hours when rescheduling an exam or hygiene appointment and 72 hours when rescheduling a surgical appointment. A charge may be applied for an appointment what is not rescheduled within this time frame or has been rescheduled twice.

Insurance Information

We submit to all insurance carriers on behalf of our patients as an “out of network” providor. Many of our patients do receive insurance coverage, however, most dental insurance policies are limited and often only pay for a portion of the procedure(s) that may need to be done.

At the initial consultation appointment, we will provde an estimate of the costs for your procedure. We will also submit an Estimation of Benefits (EOB) to your Insurance company to get a written estimate of their coverage, but this may take 4-6 weeks for a response and therefore delay any scheduling. Please be aware that there can be a variance between the amount stated on the EOB and what your insurance company will ultimately pay.
